 Connective Services provides NDIS-funded Support Coordination services to support participants to implement their plans, connect with appropriate supports, and navigate the NDIS with clarity and confidence.

Our services are delivered in line with NDIS guidelines and are focused on ethical practice, safeguarding, and participant wellbeing.

Support Coordination assists participants to understand and implement their NDIS plan while building capacity to manage supports over time.

This service may include:

• Understanding funded supports and plan inclusions
• Connecting with suitable service providers
• Coordinating services across multiple supports
• Monitoring the effectiveness of supports
• Assisting with plan utilisation and adjustments
• Supporting transitions and changes in circumstances
   

Support Coordination is delivered in a way that promotes participant choice, informed decision-making, and increasing independence where appropriate.

 Specialist Support Coordination is designed for participants with more complex needs, higher levels of risk, or multiple service systems involved.

This service may be appropriate where participants are experiencing:

• Complex or unstable support arrangements
• Safeguarding concerns or elevated risk
• Multiple providers or service systems
• Significant changes in circumstances
• Barriers to effective service engagement
