Who We Are

 Connective Services is an independent NDIS Support Coordination provider delivering structured, ethical, and participant-centred coordination services across Australia.


We support participants with complex needs to navigate the NDIS, connect with appropriate services, and maintain safe, sustainable supports that align with their goals, capacity, and changing circumstances.


Our work is grounded in strong systems knowledge, clear communication, and proactive risk awareness — ensuring participants are not left unsupported when complexity or challenges arise.

What We Do

Support Coordination (Level 2)

We assist participants to:

  • Understand and implement their NDIS plan
  • Connect with suitable providers and service
  • Build capacity to manage supports over time
  • Navigate changes, transitions, and service challenges


Specialist Support Coordination (Level 3)

We provide specialist coordination for participants with:

  • Complex needs or high levels of risk
  •  Multiple service systems involved 
  • Safeguarding concerns or instability
  • Significant changes in circumstances
     

Our approach prioritises coordination that is responsive, transparent, and aligned with duty of care obligations.

Our Approach

At Connective Services, our approach is grounded in ethical practice, safeguarding principles, and accountability across all aspects of Support Coordination.

We recognise that participants with complex needs often engage with multiple providers and systems, and that gaps in communication or oversight can place individuals at risk. Our role is to ensure coordination is structured, responsive, and informed by a clear understanding of participant needs, capacity, and changing circumstances.


We work proactively to:

  • Identify risks early by monitoring changes in circumstances, service delivery, and participant wellbeing
  • Support appropriate escalation when concerns arise, ensuring issues are addressed through the correct pathways and in a timely manner
  • Facilitate clear and consistent communication between participants, families, providers, and allied health professionals
  • Ensure supports remain suitable and safe, adapting coordination approaches as needs evolve
     

Our approach prioritises collaboration, clarity, and consistency. We work alongside participants and their informal supports to promote informed decision-making, strengthen understanding of available options, and support continuity of care.


By maintaining a strong focus on safeguarding and accountability, we aim to reduce service breakdowns, prevent escalation of risk, and support participants to achieve stability and meaningful progress toward their goals.
